Pay-Per-View Instruction and Video Release Form
Shoot 30 minutes or more of you wrestling, mail in with this release form, and make $5 per PPV! (Ex: 100 PPV= $500)

I would like to enter the following DVD titles or videos below for inclusion on WrestleMen TV. I agree that I am the sole copyright owner of these materials, and that allowing the sale of these materials on WrestleMen TV does not infringe on anyone else's rights to these materials, those appearing in the videos, nor is in violation of any agreements made prior with other companies or people appearing in such materials. I agree not to submit any materials that include full nudity, or display any sexual acts whatsoever. All participants in the videos and myself are 18 or older, and I have on file signed release statements from all participants to sell the videos.

LENGTH: Videos must be no shorter than 30 minutes. Pause camera during wrestling breaks. Longer programs may be broken down into approximately 30 min titles for PPV purposes at our discretion.

PAYMENT:
Each month a total payment of $5 per PPV purchase times the number of total purchases will be credited to your account. For instance, if all of your titles generate 100 PPV purchases, you will be paid $500. A single check will be mailed to the address provided below, or payment can be made direct via a PayPal account. You are responsible for sharing that with any of the participants in the videos. The number of PPV streams per title will also be included along with the payment.
